Section 3 — Financial Commitment Acknowledgment

One-Time Registration Fee: PHP 1,500.00 — payable upon joining. Covers PHP 150 initial membership, PHP 750 official uniform, and PHP 600 advance for the first month's training dues.

Monthly Training Dues: PHP 600.00 — strictly due on the 5th of every month.

Late Penalties: A 5-day grace period is provided. Payments after the 10th of the month incur a 5% daily penalty. Unpaid dues result in temporary suspension of training and match privileges.

Section 4 — The PCFC Member's Pledge & Waivers

The “School First” Policy

I understand that academic responsibilities take absolute precedence over football. I will support the club if my child's training load needs adjusting due to declining grades.

The Technical Firewall

I agree that all sporting decisions (tactics, playing time, and player selection) reside exclusively with the Coaches. I will not engage in sideline coaching, nor will I publicly criticize coaches, referees, or opposing teams.

Medical Waiver

I acknowledge the inherent physical risks of football. I release PCFC, its officers, and coaches from liability for injuries, and I grant permission for the staff to seek emergency medical treatment for my child if I cannot be reached.

Media & Data Privacy Consent

I grant PCFC permission to use photos/videos of my child for official club marketing. I also consent to the secure collection of my child's birth certificate, my child's ID (School ID acceptable), and my valid Parent/Guardian ID for official league and tournament registration purposes, in compliance with the Data Privacy Act.

After submission you'll receive payment instructions and a checklist for the PSA birth certificate, player ID, and parent/guardian ID required by league registration.
